Measuring harm and informing quality improvement in the Welsh NHS: the longitudinal Welsh national adverse events study.
About 10% of patients admitted to Welsh hospitals in 2011–15 were found to have suffered harm, half of those avoidably; and methods were developed for ongoing harm surveillance.
