Hypogammaglobulinemia characterizes the most common primary immunodeficiency of clinical significance: common variable immunodeficiency (CVID). In this issue Almejún et al (p 913) evaluate different qualitative aspects of the antibodies produced by patients with CVID. One of them, somatic hypermutation (SHM), introduces point mutations into the variable region of immunoglobulin genes, thus increasing antibody affinity for the antigen.
