A rare case of tri-compartmental arachnoid cyst spanning the infra-tentorial, supra-tentorial and extra-cranial compartment and presenting as posterior parietal meningocele.

Abstract
Arachnoid cysts constitute mere 1% of all the non-traumatic intracranial space occupying lesions and only 10% of these are present in the infra-tentorial compartment. We hereby report a case of a ten month-old female child with a tri-compartmental arachnoid cyst. It spanned both the infra-tentorial and supra-tentorial compartment of the intracranial space. In addition to this, it was also herniating into the extra-cranial compartment through a congenital defect in the parietal bone. The child was brought to us with a disfiguring swelling over her scalp which was progressively increasing in size. We managed the patient by excision of the extra-cranial part and placed a cysto-peritoneal shunt to take care of the intracranial part. A follow-up Magnetic Resonance Imaging (MRI) after 6 months of intervention showed a significant reduction in the size of the intracranial portion of the cyst with no recurrence of the extra-cranial part.
