Arrive: A retrospective registry of Indian patients with venous thromboembolism

Background and Aim: There is lack of substantial Indian data on venous thromboembolism (VTE). The aim of this study was to provide real-world information on patient characteristics, management strategies, clinical outcomes, and temporal trends in VTE. Subjects and Methods: Multicentre retrospective registry involving 549 medical records of patients with confirmed diagnosis of VTE (deep vein thrombosis [DVT] confirmed by Doppler ultrasonography; pulmonary embolism [PE] by computed tomography, pulmonary angiography and/or V/Q scan) from 2006 to 2010 at three Indian tertiary care hospitals. Results: Acute DVT without PE, acute DVT with PE, and PE alone were reported in 64% (352/549), 23% (124/549), and 13% (73/549) patients, respectively. Mean age was 47 (±16) years, and 70% were males. H/o DVT (34%), surgery including orthopedic surgery (28%), trauma (16%), and immobilization >3 days (14%) were the most common risk factors for VTE. Hypertension (25%), diabetes (19%), and neurological disease (other than stroke) (8%) were the most common co-morbidities. Most (94%) were treated with heparin alone (82%) or fondaparinux (2%) for initial anticoagulation; low molecular weight heparin alone (5%) or warfarin/acenocoumarol (76%) for long-term anticoagulation. Anticoagulant treatment was stopped because of bleeding in 2% (9/515) patients. Mortality was 7% among patients diagnosed with VTE during hospital stay versus 1% in those hospitalized with diagnosed VTE. The annual incidence of DVT (±PE) increased from 2006 to 2010. Conclusion: Acute DVT alone was responsible for the substantial burden of VTE in Indian patients. Bleeding was not the limiting factor for anticoagulant treatment in most patients.

Do not resuscitate: An expanding role for critical care response team

Background: Do not resuscitate (DNR) order is an important aspect of medical practice. Since the implementation of critical care response team (CCRT), frequently we have encountered with patients in the wards that should have been made DNR. Initiating DNR became an important part of CCRT activity. We were obliged to extended the role of CCRT – beyond managing seriously ill patients – in addressing the code status for patients after discussion with the managing teams. Purpose: We compare the trend of initiation of DNR orders in the regular ward before and after implementing CCRT. Methods: Our hospital is 1200 bed tertiary care center. CCRT has been launched in January 1, 2008. The CCRT is 24/7 service led by in-house North American certified intensivists. Cohort analysis of prospectively collected data of 5406 CCRT activation from January 1, 2008, to September 30, 2013. Data before implementation of CCRT was available for 299 patients from the period of June 1, 2007, to December 31, 2007. A comparison made between the two groups (before and after implementation of CCRT) for demographic data and percentage of patients in whom DNR order initiated. Results: Before CCRT implementation, 299 patients were attended by Intensive Care Unit physician for regular consultation, 41.1% were females and 52.4% were males with mean of age 58.44 ± 18.47 standard deviation (SD). DNR was initiated in 2.7% of patients. After CCRT implementation, 5904 CCRT activations, 47.6% females and 52.4% males with mean of age 59.17 ± 20.07 SD DNR initiated in 468 (7.9%) of cases. There was 5.2% increase in DNR orders initiation and completion after CCRT introduced to our institute. Conclusion: CCRT plays an important role in addressing and initiating DNR.

Outcome and predictors of cardiopulmonary resuscitation among patients admitted in Medical Intensive Care Unit in North India

Background: Outcome and predictors of survival after cardiopulmonary resuscitation (CPR) in Intensive Care Units (ICUs) have been extensively studied in western world, but data from developing countries is sparse. Objectives: To study the outcome and predictors of survival after CPR in a Medical ICU (MICU) of a tertiary level teaching hospital in North India. Materials and Methods: A 1-year prospective cohort study. Results: Of 105 in-MICU CPRs, forty patients (38.1%) achieved return of spontaneous circulation (ROSC). Only one patient (0.9%) survived up to hospital discharge. The predictors of ROSC were ventricular tachycardia/ventricular fibrillation as first monitored rhythm, intubation during CPR and CPR duration ≤ 10 min. CPR duration > 10 min was a significant factor for resuscitation failure. Conclusions: The rate of survival to hospital discharge after in-MICU CPRs is extremely poor. Our data may aid treating physicians, resuscitation teams, and families in understanding the likely outcome of patients after in-MICU CPRs.

Inadvertent migration of guidewire into Murphy’s eye of endotracheal tube during percutaneous dilatational tracheostomy

Percutaneous dilatational tracheostomy is a commonly performed bedside procedure in the Intensive Care Unit. Although serious and fatal complications have been reported, the procedure is by and large safe to perform in experienced hands. We report here an innocuous problem encountered twice. After the guidewire insertion and dilatation, subsequent railroading became difficult owing to migration of guidewire into the Murphy's eye of the endotracheal tube (ETT). Awareness about this possibility can avert inadvertent delays and complications during the procedure. A tug or gentle pulling of ETT after insertion of the guidewire rules out an impaction in the eye or other part of the ETT.

An obstruction not to forget: Pseudo-obstruction (Ogilvie syndrome): Single center experience

Purpose: Colonic pseudo obstruction disease commonly seen in the elderly, immobile patient group can cause serious mortality and morbidity. Our objective in this retrospective study is to share our clinical experience by evaluating patients with Ogilvie syndrome who were followed and treated in our clinic. Methods: Eleven cases with the diagnosis of Ogilvie syndrome followed up and treated between September 2010 and April 2013 were evaluated retrospectively. All the patients that had no symptoms of acute abdominal pain were initiated conservative treatment. Colonoscopic decompression was attempted for patients whose clinical pictures were not recovered. Patients underwent operation if they developed peritoneal irritation symptoms during these procedures and of their number of white blood cells seriously increased during the follow-up period. Results: A total of 11 patients were treated for Ogilvie syndrome. 6 of the patients underwent surgical treatment, and 5 were treated medically. Mortality developed in two patients. The main cause of mortality was a delay in diagnosis and additional severe underlying diseases. Seven patients were given Neostigmine. Of these, 2 patients required surgery and 3 patients responded to Neostigmine. Conclusion: Ogilvie syndrome is a rare cause of ileus of the colon. It is more common particularly in old patients with additional problems. If the disease is suspected and diagnosed early, unnecessary surgical interventions can be prevented with medical treatment choices.

The current article deals with the art and science of calcium correction in pancreatitis patients.

Lung atelectasis resulting after cardiopulmonary bypass (CPB) can result in increased intrapulmonary shunting and consequent hypoxemia. Advanced pressure control modes of ventilation might have at least a theoretical advantage over conventional modes by assuring a minimum target tidal volume delivery at reasonable pressures, thus having potential advantages while ventilating patients with pulmonary atelectasis postcardiac surgery. However, the utility of these modes in the post-CPB setting have not been widely investigated, and their role in cardiac intensive care, therefore, remains quite limited.

Acute respiratory distress syndrome (ARDS) is characterized by acute onset respiratory failure with bilateral pulmonary infiltrates and hypoxemia. Current evidence suggests different respiratory mechanics in pulmonary ARDS (ARDSp) and extrapulmonary ARDS (ARDSexp) with disproportionate decrease in lung compliance in the former and chest wall compliance in the latter. Herein, we report two patients of ARDS, one each with ARDSp and ARDSexp that were managed using real-time esophageal pressure monitoring using the AVEA ventilator to tailor the ventilatory strategy.

Brain lesions in eclampsia: A series of 39 cases admitted in an Intensive Care Unit

The aim of this study was to identify the encephalic lesions in the eclampsia occurrences. Within a period of 18 months, computed tomography (CT) of the brain was performed in all patients admitted in intensive care for eclampsia. These CTs were analyzed and intracerebral lesions were identified. Thirty-nine patients were included. We noted 10 cases of ischemic stroke, 9 cases of cerebral edema, and 3 cases of hemorrhagic stroke and subarachnoid hemorrhage. The CT scan came back to normal in 20 eclamptic patients. Overall, delays in obstetric and intensive care and time of completion of the CT were long. CT has allowed highlighting in patients with eclampsia varied intracerebral lesions. The early performance of the CT is therefore essential for a better support of patients.

“I Always Worry about What Might Happen Ahead”: Implementing Safer Conception Services in the Current Environment of Reproductive Counseling for HIV-Affected Men and Women in Uganda

Background. We explored healthcare provider perspectives and practices regarding safer conception counseling for HIV-affected clients. Methods. We conducted semistructured interviews with 38 providers (medical and clinical officers, nurses, peer counselors, and village health workers) delivering care to HIV-infected clients across 5 healthcare centres in Mbarara District, Uganda. Interview transcripts were analyzed using content analysis. Results. Of 38 providers, 76% were women with median age 34 years (range 24–57). First, we discuss providers' reproductive counseling practices. Emergent themes include that providers (1) assess reproductive goals of HIV-infected female clients frequently, but infrequently for male clients; (2) offer counseling focused on "family planning" and maternal and child health; (3) empathize with the importance of having children for HIV-affected clients; and (4) describe opportunities to counsel HIV-serodiscordant couples. Second, we discuss provider-level challenges that impede safer conception counseling. Emergent themes included the following: (1) providers struggle to translate reproductive rights language into individualized risk reduction given concerns about maternal health and HIV transmission and (2) providers lack safer conception training and support needed to provide counseling. Discussion. Tailored guidelines and training are required for providers to implement safer conception counseling. Such support must respond to provider experiences with adverse HIV-related maternal and child outcomes and a national emphasis on pregnancy prevention.

Materials, Vol. 9, Pages 179: Compositional Design of Dielectric, Ferroelectric and Piezoelectric Properties of (K, Na)NbO3 and (Ba, Na)(Ti, Nb)O3 Based Ceramics Prepared by Different Sintering Routes

Lead free piezoelectric materials are being intensively investigated in order to substitute lead based ones, commonly used in many different applications. Among the most promising lead-free materials are those with modified NaNbO3, such as (K, Na)NbO3 (KNN) and (Ba, Na)(Ti, Nb)O3 (BTNN) families. From a ceramic processing point of view, high density single phase KNN and BTNN ceramics are very difficult to sinter due to the volatility of the alkaline elements, the narrow sintering temperature range and the anomalous grain growth. In this work, Spark Plasma Sintering (SPS) and high-energy ball milling (HEBM), following heat treatments (calcining and sintering), in oxidative (O2) atmosphere have been used to prepare single phase highly densified KNN ("pure" and Cu2+ or Li1+ doped), with theoretical densities ρth > 97% and BTNN ceramics (ρth - 90%), respectively. Using BTTN ceramics with a P4mm perovskite-like structure, we showed that by increasing the NaNbO3 content, the ferroelectric properties change from having a relaxor effect to an almost "normal" ferroelectric character, while the tetragonality and grain size increase and the shear piezoelectric coefficients (k15, g15 and d15) improve. For KNN ceramics, the results reveal that the values for remanent polarization as well as for most of the coercive field are quite similar among all compositions. These facts evidenced that Cu2+ may be incorporated into the A and/or B sites of the perovskite structure, having both hardening and softening effects.

Chromatography, Vol. 3, Pages 9: The Simultaneous Determination of Silicic, Boric and Carbonic Acids in Natural Water via Ion-Exclusion Chromatography with a Charged Aerosol Detector

The simple and simultaneous determination of silicic, boric and carbonic acids was made using ion-exclusion chromatography (IEC) and a Corona™ charged aerosol detector (C-CAD). Silicic and boric acids were separated by the column packed with a weakly acidic cation-exchange resin in H+-form and ultra-pure water eluent, and the detector responses were improved by the addition of acetonitrile to eluent. Under the optimized conditions, the simultaneous determination of weak inorganic acids, except for carbonic acid, was successfully performed. When the conversion column packed with a strong acidic cation-exchange resin in Na+- or K+-form was inserted between the separation column and the detector, weak inorganic acids including carbonic acid could be detected by the C-CAD. The calibration curves were linear in the range of 0.5–10 mg·L−1 as Si for silicic acid (r2 = 0.996), 10–100 mg·L−1 as B for boric acid (r2 = 0.998) and 1.3–21 mg·L−1 as C for carbonic acid (r2 = 0.993). The detection limits based on three times the standard deviation were 0.03 mg·L−1 as Si for silicic acid, 0.40 mg·L−1 as B for boric acid and 0.08 mg·L−1 as C for carbonic acid. This method was applicable to river, hot spring and drinking water.

Applied Sciences, Vol. 6, Pages 76: Investigation of the Machining Stability of a Milling Machine with Hybrid Guideway Systems

This study was aimed to investigate the machining stability of a horizontal milling machine with hybrid guideway systems by finite element method. To this purpose, we first created finite element model of the milling machine with the introduction of the contact stiffness defined at the sliding and rolling interfaces, respectively. Also, the motorized built-in spindle model was created and implemented in the whole machine model. Results of finite element simulations reveal that linear guides with different preloads greatly affect the dynamic responses and machining stability of the horizontal milling machine. The critical cutting depth predicted at the vibration mode associated with the machine tool structure is about 10 mm and 25 mm in the X and Y direction, respectively, while the cutting depth predicted at the vibration mode associated with the spindle structure is about 6.0 mm. Effects of Ca content (in the reactant mixture) on the formation and the photoluminescence properties of CaAlSiN3:Eu2+ phosphor (CASIN) were investigated by a combustion synthesis method. Ca, Al, Si, Eu2O3, NaN3, NH4Cl and Si3N4 powders were used as the starting materials and they were mixed and pressed into a compact which was then wrapped up with an igniting agent (i.e., Mg + Fe3O4). The compact was ignited by electrical heating under a N2 pressure of ≤1.0 MPa. By keeping the molar ratios of Al and Si (including the Si powder and the Si in Si3N4 powder) both at 1.00 and that of Eu2O3 at 0.02, XRD (X-ray diffraction) coupled with TEM-EDS (transmission electron microscope equipped with an energy-dispersive X-ray spectroscope) and SAED (selected area electron diffraction) measurements show that AlN:Eu2+ and Ca-α-SiAlON:Eu2+ are formed as the major phosphor products when the Ca molar ratio (denoted by Y) is equal to 0.25 and AlN:Eu2+ and Ca-α-SiAlON:Eu2+ could not be detected at Y ≥ 0.75 and ≥1.00, respectively. CASIN (i.e., CaAlSiN3:Eu2+) becomes the only phosphor product as Y is increased to 1.00 and higher. The extent of formation of CASIN increases with increasing Y up to 1.50 and begins to decrease as Y is further increased to 1.68. While the excitation wavelength regions are similar at various Y, the emission wavelength regions vary significantly as Y is increased from 0.25 to 1.00 due to different combinations of phosphor phases formed at different Y. The emission intensity of CASIN was found to vary with Y in a similar trend to its extent of formation. The Ca and Eu contents (expressed as molar ratios) in the synthesized products were found to increase roughly with increasing Y but were both lower than the respective Ca and Eu contents in the reactant mixtures.

“I Always Worry about What Might Happen Ahead”: Implementing Safer Conception Services in the Current Environment of Reproductive Counseling for HIV-Affected Men and Women in Uganda

Background. We explored healthcare provider perspectives and practices regarding safer conception counseling for HIV-affected clients. Methods. We conducted semistructured interviews with 38 providers (medical and clinical officers, nurses, peer counselors, and village health workers) delivering care to HIV-infected clients across 5 healthcare centres in Mbarara District, Uganda. Interview transcripts were analyzed using content analysis. Results. Of 38 providers, 76% were women with median age 34 years (range 24–57). First, we discuss providers' reproductive counseling practices. Emergent themes include that providers (1) assess reproductive goals of HIV-infected female clients frequently, but infrequently for male clients; (2) offer counseling focused on "family planning" and maternal and child health; (3) empathize with the importance of having children for HIV-affected clients; and (4) describe opportunities to counsel HIV-serodiscordant couples. Second, we discuss provider-level challenges that impede safer conception counseling. Emergent themes included the following: (1) providers struggle to translate reproductive rights language into individualized risk reduction given concerns about maternal health and HIV transmission and (2) providers lack safer conception training and support needed to provide counseling. Discussion. Tailored guidelines and training are required for providers to implement safer conception counseling. Such support must respond to provider experiences with adverse HIV-related maternal and child outcomes and a national emphasis on pregnancy prevention.

Lead free piezoelectric materials are being intensively investigated in order to substitute lead based ones, commonly used in many different applications. Among the most promising lead-free materials are those with modified NaNbO3, such as (K, Na)NbO3 (KNN) and (Ba, Na)(Ti, Nb)O3 (BTNN) families. From a ceramic processing point of view, high density single phase KNN and BTNN ceramics are very difficult to sinter due to the volatility of the alkaline elements, the narrow sintering temperature range and the anomalous grain growth. In this work, Spark Plasma Sintering (SPS) and high-energy ball milling (HEBM), following heat treatments (calcining and sintering), in oxidative (O2) atmosphere have been used to prepare single phase highly densified KNN ("pure" and Cu2+ or Li1+ doped), with theoretical densities ρth > 97% and BTNN ceramics (ρth - 90%), respectively. Using BTTN ceramics with a P4mm perovskite-like structure, we showed that by increasing the NaNbO3 content, the ferroelectric properties change from having a relaxor effect to an almost "normal" ferroelectric character, while the tetragonality and grain size increase and the shear piezoelectric coefficients (k15, g15 and d15) improve. For KNN ceramics, the results reveal that the values for remanent polarization as well as for most of the coercive field are quite similar among all compositions. These facts evidenced that Cu2+ may be incorporated into the A and/or B sites of the perovskite structure, having both hardening and softening effects.

The simple and simultaneous determination of silicic, boric and carbonic acids was made using ion-exclusion chromatography (IEC) and a Corona™ charged aerosol detector (C-CAD). Silicic and boric acids were separated by the column packed with a weakly acidic cation-exchange resin in H+-form and ultra-pure water eluent, and the detector responses were improved by the addition of acetonitrile to eluent. Under the optimized conditions, the simultaneous determination of weak inorganic acids, except for carbonic acid, was successfully performed. When the conversion column packed with a strong acidic cation-exchange resin in Na+- or K+-form was inserted between the separation column and the detector, weak inorganic acids including carbonic acid could be detected by the C-CAD. The calibration curves were linear in the range of 0.5–10 mg·L−1 as Si for silicic acid (r2 = 0.996), 10–100 mg·L−1 as B for boric acid (r2 = 0.998) and 1.3–21 mg·L−1 as C for carbonic acid (r2 = 0.993). The detection limits based on three times the standard deviation were 0.03 mg·L−1 as Si for silicic acid, 0.40 mg·L−1 as B for boric acid and 0.08 mg·L−1 as C for carbonic acid. This method was applicable to river, hot spring and drinking water.

This study was aimed to investigate the machining stability of a horizontal milling machine with hybrid guideway systems by finite element method. To this purpose, we first created finite element model of the milling machine with the introduction of the contact stiffness defined at the sliding and rolling interfaces, respectively. Also, the motorized built-in spindle model was created and implemented in the whole machine model. Results of finite element simulations reveal that linear guides with different preloads greatly affect the dynamic responses and machining stability of the horizontal milling machine. The critical cutting depth predicted at the vibration mode associated with the machine tool structure is about 10 mm and 25 mm in the X and Y direction, respectively, while the cutting depth predicted at the vibration mode associated with the spindle structure is about 6.0 mm. Also, the machining stability can be increased when the preload of linear roller guides of the feeding mechanism is changed from lower to higher amount.

Subsurface drainage is a prerequisite to grow winter crops in the consolidated paddy fields in Northern Iran. A four-year study (2011–2015) was conducted to quantify the effects of subsurface drainage on the saturated hydraulic conductivity, water table, drain discharge and winter crop yields. Subsurface drainage systems with two drain depths of 0.65 and 0.90 m and two drain spacings of 15 and 30 m were installed at the consolidated paddy fields of Sari Agricultural Sciences and Natural Resources University, Iran. During four successive winter seasons, the water table depth and drain discharge were measured daily. Soil saturated hydraulic conductivity was measured twice; before drainage system installation and four years following the installation. Canola grain yields were determined at harvest of each cultivation season. During the study period, the soil saturated hydraulic conductivity increased with the highest increase in the top 0–30 cm. The deeper drains were more effective in controlling the water table compared to the shallow, and the daily drain discharge of the deeper drains in the fourth year were higher than those of shallow drains. The canola grain yield of all drainage systems increased significantly by the seasons, and the largest difference in canola grain yield between first and fourth seasons was 2191 kg·ha−1 (318% increase) in the fields with 0.90 m drain depth and 30 m drain spacing. Totally, it became clear that installation of subsurface drainage systems with 0.90 m depth and 30 m spacing in the paddy fields of Northern Iran can be recommended to achieve high yield of winter crop, soil condition improvement, and multi-purpose land use.

Forests, Vol. 7, Pages 61: Estimation of Tree Stem Attributes Using Terrestrial Photogrammetry with a Camera Rig

We propose a novel photogrammetric method for field plot inventory, designed for simplicity and time efficiency on-site. A prototype multi-camera rig was used to acquire images from field plot centers in multiple directions. The acquisition time on-site was less than two minutes. From each view, a point cloud was generated using a novel, rig-based matching of detected SIFT keypoints. Stems were detected in the merged point cloud, and their positions and diameters were estimated. The method was evaluated on 25 hemi-boreal forest plots of a 10-m radius. Due to difficult lighting conditions and faulty hardware, imagery from only six field plots was processed. The method performed best on three plots with clearly visible stems with a 76% detection rate and 0% commission. Diameters could be estimated for 40% of the stems with an RMSE of 2.8–9.5 cm. The results are comparable to other camera-based methods evaluated in a similar manner. The results are inferior to TLS-based methods. However, our method is easily extended to multiple station image schemas, something that could significantly improve the results while retaining low commission errors and time on-site. Furthermore, with smaller hardware, we believe this could be a useful technique for measuring stem attributes in the forest.

Sequential Monte Carlo (SMC) methods are widely used for non-linear filtering purposes. However, the SMC scope encompasses wider applications such as estimating static model parameters so much that it is becoming a serious alternative to Markov-Chain Monte-Carlo (MCMC) methods. Not only do SMC algorithms draw posterior distributions of static or dynamic parameters but additionally they provide an estimate of the marginal likelihood. The tempered and time (TNT) algorithm, developed in this paper, combines (off-line) tempered SMC inference with on-line SMC inference for drawing realizations from many sequential posterior distributions without experiencing a particle degeneracy problem. Furthermore, it introduces a new MCMC rejuvenation step that is generic, automated and well-suited for multi-modal distributions. As this update relies on the wide heuristic optimization literature, numerous extensions are readily available. The algorithm is notably appropriate for estimating change-point models. As an example, we compare several change-point GARCH models through their marginal log-likelihoods over time.

Currently, the environment and sustainability are important topics for every industry. The food industry is particularly complicated in this regard because of the dynamic and complex character of food products and their production. This study uses structural equation modeling (SEM) and a fuzzy analytic hierarchy process (FAHP) to investigate which factors are suitable for evaluating the environmental performance of Thailand's food industry. A first-stage questionnaire survey was conducted with 178 managers in the food industry that obtained a certificate from the Department of Industrial Work of Thailand to synthesize the performance measurement model and the significance of the relationship between the indicators. A second-stage questionnaire measured 18 experts' priorities regarding the criteria and sub-factors involved in the different aspects and assessment items regarding environmental performance. SEM showed that quality management, market orientation, and innovation capability have a significantly positive effect on environmental performance. The FAHP showed that the experts were most concerned about quality management, followed by market orientation and innovation capability; the assessment items for quality policy, quality assurance, and customer orientation were of the most concern. The findings of this study can be referenced and support managerial decision making when monitoring environmental performance.

Episodic recharge as a result of infrequent, high intensity precipitation events comprises the bulk of groundwater recharge in arid environments. Climate change and shifts in precipitation intensity will affect groundwater continuity, thus altering groundwater recharge. This study aims to identify changes in the ratio of groundwater recharge and precipitation, the R:P ratio, in the arid southwestern United States to characterize observed changes in groundwater recharge attributed to variations in precipitation intensity. Our precipitation metric, precipitation intensity magnification, was used to investigate the relationship between the R:P ratio and precipitation intensity. Our analysis identified significant changes in the R:P ratio concurrent with decreases in precipitation intensity. The results illustrate the importance of precipitation intensity in relation to groundwater recharge in arid regions and provide further insights for groundwater management in nonrenewable groundwater systems and in a changing climate.

The evolution of the entropy production in solids due to heat transfer is usually associated with the Prigogine's minimum entropy production principle. In this paper, we propose a critical review of the results of Prigogine and some comments on the succeeding literature. We suggest a characterization of the evolution of the entropy production of the system through the generalized Fourier modes, showing that they are the only states with a time independent entropy production. The variational approach and a Lyapunov functional of the temperature, monotonically decreasing with time, are discussed. We describe the analytic properties of the entropy production as a function of time in terms of the generalized Fourier coefficients of the system. Analytical tools are used throughout the paper and numerical examples will support the statements.

Vulva cancer is a relatively rare disease; among vulvar cancers, the most common histological type is squamous cell carcinoma (SCC). Surgery, based on individual treatment needs, is the first-line therapeutic option; less radical surgery can be used in early-stage disease, plastic reconstructive techniques can be used for large defects, and radiation therapy can be employed for locoregional involvement. Reconstructive procedure options following surgery have traditionally involved the use of skin grafts and skin, fasciocutaneous, or myocutaneous flaps [1]. Preparation of decarboxylic-functionalized weak cation exchanger and application for simultaneous separation of alkali, alkaline earth and transition metals

A novel weak cation exchanger (WCX) with dicarboxyl groups functionalized has been developed by clicking mercaptosuccinic acid onto silica gel. The simple synthesis starts with modification of silica gel with triethoxyvinylsilane, followed by efficient coupling vinyl-bonded silica with mercaptosuccinic acid via a "thiol-ene" click reaction. The obtained WCX demonstrated good separation and high selectivity towards common metals. Simultaneous separation of 10 alkali, alkaline earth and transition metals was achieved within 12min. Ion exchange and complex mechanism dominates the separation process. Its utility was demonstrated for determination of metals in tap water.

Materials, Vol. 9, Pages 173: Effect of Initial Aluminum Alloy Particle Size on the Damage of Carbon Nanotubes during Ball Milling

Damage to carbon nanotubes (CNTs) during the fabrication process of CNT reinforced composites has great influence on their mechanical properties. In this study, the 2014 Al with powder sizes of 20, 9 and 5 μm was selected to study the effect of initial particle size on the damage to carbon nanotubes (CNTs) during ball milling. The result shows that for CNTs in the ball milled CNT/Al (with powder size of 20 and 9 μm) mixtures, the intensity ratio of the D band and the G band (ID/IG) first increased and then reached a plateau, mainly because most of the CNTs are embedded, to a certain extent, in the aluminum powder after milling, which could protect the CNTs from damage during further milling. While for CNTs in the ball milled CNT/Al (with powder size of 5 μm) mixture, the ID/IG ratio continues to climb from 1.31 to 2.33 with time, indicating continuous damage to the CNTs occurs during the milling. Differential scanning calorimetry (DSC) analysis demonstrates that the chemical instability increased with an increase in the damage level of CNTs, resulting in the formation of aluminum carbide (Al4C3) at a lower temperature before the melting of aluminum, which is detrimental to their mechanical properties.

Cells, Vol. 5, Pages 11: Rab GTPases and the Autophagy Pathway: Bacterial Targets for a Suitable Biogenesis and Trafficking of Their Own Vacuoles

Autophagy is an intracellular process that comprises degradation of damaged organelles, protein aggregates and intracellular pathogens, having an important role in controlling the fate of invading microorganisms. Intracellular pathogens are internalized by professional and non-professional phagocytes, localizing in compartments called phagosomes. To degrade the internalized microorganism, the microbial phagosome matures by fusion events with early and late endosomal compartments and lysosomes, a process that is regulated by Rab GTPases. Interestingly, in order to survive and replicate in the phagosome, some pathogens employ different strategies to manipulate vesicular traffic, inhibiting phagolysosomal biogenesis (e.g., Staphylococcus aureus and Mycobacterium tuberculosis) or surviving in acidic compartments and forming replicative vacuoles (e.g., Coxiella burnetti and Legionella pneumophila). The bacteria described in this review often use secretion systems to control the host's response and thus disseminate. To date, eight types of secretion systems (Type I to Type VIII) are known. Some of these systems are used by bacteria to translocate pathogenic proteins into the host cell and regulate replicative vacuole formation, apoptosis, cytokine responses, and autophagy. Herein, we have focused on how bacteria manipulate small Rab GTPases to control many of these processes. The growing knowledge in this field may facilitate the development of new treatments or contribute to the prevention of these types of bacterial infections.

JCM, Vol. 5, Pages 35: Kidney Function as a Determinant of HDL and Triglyceride Concentrations in the Australian Population

Background: Chronic kidney disease (CKD) is a potent risk factor for cardiovascular disease (CVD). CVD risk increases in a stepwise manner with increasing kidney impairment and is significantly reduced by kidney transplantation, suggesting a causal relationship. Dyslipidemia, a well recognised CVD risk factor, is highly prevalent in CKD. While dyslipidemia is a risk factor for CKD, kidney impairment can also induce a dyslipidemic state that may contribute to the excess burden of CVD in CKD. We utilised a multipronged approach to determine whether a causal relationship exists. Materials and Methods: Retrospective case-control analysis of 816 patients admitted to the Royal Hobart Hospital in 2008–2009 with different degrees of kidney impairment and retrospective before-after cohort analysis of 60 patients who received a transplanted kidney between 1999 and 2009. Results: Decreased estimated GFR (eGFR) was independently associated with decreased high density lipoprotein (HDL, p < 0.0001) and increased triglyceride concentrations (p < 0.01) in multivariate analysis. There was no significant relationship between eGFR and low density lipoprotein (LDL) or total cholesterol in multivariate analysis. Kidney transplantation increased HDL (p < 0.0001) and decreased triglyceride (p = 0.007) concentration, whereas there was no significant change in LDL and total cholesterol. These effects were dependent on maintenance of graft function, statin therapy (those who were on) if graft failure occurred then HDL again decreased and triglycerides increased. Conclusions: Kidney transplantation ameliorated alterations in plasma lipoprotein profile associated with kidney impairment, an effect that was dependent on the maintenance of graft function. These data suggest that kidney function is a determinant of HDL and triglyceride concentrations in patients with CKD.

Toxins, Vol. 8, Pages 67: Perfringolysin O Theta Toxin as a Tool to Monitor the Distribution and Inhomogeneity of Cholesterol in Cellular Membranes

Cholesterol is an essential structural component of cellular membranes in eukaryotes. Cholesterol in the exofacial leaflet of the plasma membrane is thought to form membrane nanodomains with sphingolipids and specific proteins. Additionally, cholesterol is found in the intracellular membranes of endosomes and has crucial functions in membrane trafficking. Furthermore, cellular cholesterol homeostasis and regulation of de novo synthesis rely on transport via both vesicular and non-vesicular pathways. Thus, the ability to visualize and detect intracellular cholesterol, especially in the plasma membrane, is critical to understanding the complex biology associated with cholesterol and the nanodomains. Perfringolysin O (PFO) theta toxin is one of the toxins secreted by the anaerobic bacteria Clostridium perfringens and this toxin forms pores in the plasma membrane that causes cell lysis. It is well understood that PFO recognizes and binds to cholesterol in the exofacial leaflets of the plasma membrane, and domain 4 of PFO (D4) is sufficient for the binding of cholesterol. Recent studies have taken advantage of this high-affinity cholesterol-binding domain to create a variety of cholesterol biosensors by using a non-toxic PFO or the D4 in isolation. This review highlights the characteristics and usefulness of, and the principal findings related to, these PFO-derived cholesterol biosensors.

Adolescent Perceptions of Outgrowing Childhood Attention-Deficit Hyperactivity Disorder: Relationship to Symptoms and Quality of Life.

Objective: Parental and self-perceptions about outgrowing attention-deficit hyperactivity disorder (ADHD) likely impact treatment decisions; however, little is known about these perceptions. The aim of this study was to assess adolescent and parent perceptions of outgrowing childhood ADHD and evaluate how these perceptions relate to ADHD symptoms and quality of life (QoL). Method: Cross-sectional survey of adolescents (13-18 years) with a childhood diagnosis of ADHD and their parents. Participants were contacted by mail and completed questionnaires about: perceptions of outgrowing ADHD, current ADHD symptoms (the Conners' Scales), and QoL (PedsQL). ANCOVAs were conducted to assess the relationship between the perception of outgrowing ADHD and: (1) current ADHD symptoms and (2) QoL, controlling for co-morbidities and medication use. Results: 115 adolescent-parent dyads were enrolled. Mean age was 15.5 +/- 1.7 years, 73% were male, and 63% were taking ADHD medication. Only 16% of adolescents and 9% of parents reported outgrowing ADHD, with little correlation between parent and adolescent responses (kappa 0.3). Adolescents who reported Yes or Don't Know to outgrowing ADHD, compared to those who reported No, had lower scores on the inattention (IA) and hyperactivity (H) scales of both parent report and adolescent self-report Conners' scales and higher scores on the adolescent self-report PedsQL. Conclusion: A majority of adolescents do not endorse outgrowing their ADHD diagnosis, but those that do demonstrate fewer ADHD symptoms and better QoL than those who don't, suggesting that adolescents have insight about their level of impairment and should have a valued role in making treatment decisions.
