A novel microsurgery robot mechanism with mechanical motion scalability for intraocular and reconstructive surgery

xlomafota13 shared this article with you from Inoreader

Abstract

Intraocular surgery and reconstructive surgery are challenging microsurgery procedures that require two types of motion: precise motion and larger motion. To effectively perform the requisite motion using a robot, it is necessary to develop a manipulator that can adjust the scale of motion between precise motion and less precise, yet larger motion. In this paper, we propose a novel microsurgery robot using the dual delta structure to mechanically scale the motion to seamlessly adjust between precise and larger motion. The dual delta structure (DDS) forms a lever mechanism that enables the motion scaling at the end‐effecter using two delta platforms. Seamless scale adjustment enables the robot to effectively perform various surgical moves. A prototype robot system was developed to validate the effectiveness of the DDS. Through a graphical simulation and measurement experiment, the robot's precision level and attainable workspace has been confirmed adequate for intraocular and rec onstructive surgery. The experiment results in various scale settings validated the scaling mechanism of the DDS.

Overcoming Operator-Generated False-Negative Results in SARS-CoV-2 Testing

To the Editor I share the concerns of Higgins et al regarding the low sensitivity of the gold standard for detecting severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and thank them for their detailed description of how to properly obtain nasopharyngeal swabs. However, the rejection by Higgins et al of nasal swabbing as a valid method for coronavirus disease 2019 (COVID-19) testing is inconsistent with current understanding. Most peer-reviewed publications comparing nasal vs nasopharyngeal swabbing for COVID-19 specimen collection have demonstrated noninferiority between methods.

Overcoming Operator-Generated False-Negative Results in SARS-CoV-2 Testing—Reply

In Reply We thank Dr Chee for the comments about our Viewpoint. Saliva testing may have reasonable clinical acceptance in certain situations. It should be noted that it remains a less accurate method, and therefore we would not recommend it for critical situations such as preoperative testing or to determine coronavirus disease 2019 (COVID-19) status during a hospital admission. However, saliva tests might be useful in estimating the rate of positivity in a population as in children and parents in certain school districts in the US. The nasopharyngeal swab test is actually not difficult to perform if proper education is provided and, for facilities opting for nasopharyngeal testing, the purpose of our Viewpoint was to highlight the proper anatomy.

24‐Hour Ambulatory Blood Pressure Variability in Children with Obstructive Sleep Apnea

xlomafota13 shared this article with you from Inoreader

Objective

To evaluate blood pressure (BP) variability in 24‐hour ambulatory BP monitoring in children with obstructive sleep apnea (OSA).

Study Design

Case series study.

Methods

Children aged 4 to 16 years with clinical symptoms were recruited in a tertiary medical center. Overnight polysomnography and 24‐hour recordings of ambulatory BP were performed for each child. The severity of OSA was classified as primary snoring (apnea‐hypopnea index [AHI] < 1), mild OSA (1 ≤ AHI < 5), moderate OSA (10 > AHI ≥ 5), and severe OSA (AHI ≥ 10). The standard deviation of mean BP was used as an indicator of BP variability.

Results

A total of 550 children were included (mean age: 7.6 years; 70% were boys; 20% were obese). Compared with the children with primary snoring, children with severe OSA exhibited significantly higher nighttime systolic BP (108.0 vs. 100.5 mmHg, P < .001), nighttime diastolic BP (58.9 vs 55.6 mmHg, P = .002), nighttime mean arterial pressure (75.3 vs. 70.5 mmHg, P < .001), nighttime systolic BP load (40.5% vs. 25.0%, P < .001), nighttime diastolic BP load (25.3% vs. 12.9%, P < .001), and nighttime systolic BP variability (11.4 vs. 9.6, P = .001). Multiple linear regression analyses revealed an independent association between AHI and nighttime systolic BP variability (regression coefficient = 0.31, 95% CI = 0.06–0.56, P = .015) after adjustment for age, gender, adiposity, and hypertensive status.

Conclusions

OSA in children is associated with increased BP and BP variability.

Incidence and Prognosis of Brain Metastases in Head and Neck Cancer Patients at Diagnosis

xlomafota13 shared this article with you from Inoreader

Objectives

Limited data is available on head and neck cancer (HNC) patients presenting with brain metastases (BM) at initial diagnosis. We sought to evaluate the incidence, management, and treatment outcomes of these patients using the National Cancer Database (NCDB).

Methods

We identified 465,925 patients diagnosed with HNC between 2010 and 2015 in the NCDB. 14,583 of these patients presented with metastatic disease to any site. 440 of these patients had BM at the time of initial diagnosis. Overall survival was compared using the Kaplan–Meier method. Cox proportional hazard modeling, propensity score matching, and subgroup analyses were performed.

Results

The median age overall was 62.0 years. Nasopharynx NOS (13.2%) and Parotid Gland (10.9%) were the most common anatomical sites with the highest frequency of BM. The overall median survival time was 7.1 months. Predictors for the presence of BM included distant metastasis to the bone, liver, or lung on univariate analysis, and bone or lung on multivariate analysis. High‐risk Human Papilloma Virus status was associated with a lower chance of BM. No pattern was determined when comparing lymph node level involvement and BM. The median survival for patients receiving radiation therapy and multi‐agent chemotherapy was 8.4 and 11.7 months, respectively. Immunotherapy administered as first course therapy did not influence median survival. Most patients received radiation (62.7%) therapy and chemotherapy (50.2%).

Conclusions

The data extracted and analyzed from the NCDB should work to aid in the surveillance and management of BM in patients with HNC.
