Missing Source Credit for Figure and Table

In the Brief Report titled "Characterizing 18 Years of the Death With Dignity Act in Oregon," published online in JAMA Oncology on April 6, 2017, the source for the data reported in the Figure and Table was omitted. The source is the Oregon Health Authority, which is noted in the Abstract and Methods section of the article and is listed as reference 3 in the article. The Figure was reproduced from Figure 1 in reference 3. In addition, reference 3 had an incorrect title and incomplete source information, but the URL was correct. The article has been corrected online to add footnotes explaining the source for data in the Figure and the Table.
