Abstract
BACKGROUND: Intracranial dermoid cysts represent benign embryologic tumors. Children and young adults are most affected. Infected intradural dermoid cyst with complete dermal sinus of posterior fossa is exceptional. The main feature of this dermal sinus leads from the midline intradural cyst through an opening in the occipital bone to be attached to the skin.
METHOD: We report two cases of infected intradural dermoid cyst with complete dermal sinus of posterior fossa in children, with special attention to the clinical, anatomical features of this rare combination.
RESULTS: The first, five-year-old male with a median well shaped cystic lesion responsible of a triventricular hydrocephalus revealed by statokinetic cerebellar syndrome and a dermal sinus. The second, thirty-six month-old female who had a median well limited occipital cystic structure revealed by double dermal sinus. We performed before tumor excision, an external shunt to relieve the increased intracranial pressure in the first case. Direct surgical approach through a median sub-occipital craniectomy followed by systemic antibiotic therapy in the both cases was carried out. Good outcome was observed in both cases.
CONCLUSION: The infected intradural dermoid cyst with complete dermal sinus of posterior fossa is rare. A double dermal sinus is unpublished. Surgery followed by systemic antibiotic therapy lead to a good outcome.
