Late recurrence of choroid plexus carcinoma
Abstract Background Choroid plexus carcinomas (CPC) are rare malignant brain tumours arising from the choroid plexus epithelium. CPC are most common in the paediatric population, particularly those under 2 years of age. Common presentations include headache, diplopia and signs of increased intracranial pressure such as nausea and vomiting. Infants may present with increased head circumference, bulging fontanelles, splayed cranial sutures...

Compressive epidural collection as an unusual complication of FESS for a pediatric mycetoma
Abstract Functional endoscopic sinus surgery (FESS) is a treatment of choice for fungal sinus ball (mycetoma), which is considered safe with a very low major complication rate. We present an unusual case of a 12-year-old female, who underwent FESS for a sphenoid sinus mycetoma and which was complicated by an acute, compressive epidural fluid collection. This presumably resulted from sinus irrigation in the setting of an under-appreciated skull base and mucosal defects causing...

Importance of anatomical site in the metastases of auricular cutaneous squamous cell carcinoma: an observational study
Malignancies of the auricular skin tend to metastasise to preauricular, parotid, facial, and level II (possibly to levels IV and V) lymph nodes in an unpredictable manner. Over the years, we have observed that this pattern of metastasis is commonly linked to lesions that involve the lower half of the ear. To find conclusive evidence based on this hypothetical observation, we retrospectively studied 108 patients who presented with squamous cell carcinoma (SCC) of the ear, and looked at the exact auricular...

Deep Inferior Epigastric Artery Vascularized Lymph Node Transfer: A Simple and Safe Option for Lymphedema
Lymphedema is caused by lymphatic dysfunction which leads to progressive limb enlargement. Vascularized lymph node transfer (VLNT) is a microsurgical treatment for lymphedema. Frequently used VLNT donor sites may risk donor-site lymphedema (groin) or require intraperitoneal harvest (right gastroepiploic). The purpose of this study is to describe a novel VLNT using the deep inferior epigastric artery (DIEA) nodes.Patients with lymphedema who underwent VLNT using DIEA nodes were retrospectively reviewed.

Occupational barriers to HIV care in female sex workers living with HIV: structural or community solutions?
The UNAIDS 90-90-90 targets, defined as 90% of people living with HIV aware of their status, among which 90% are on antiretroviral treatment (ART) and among which 90% have HIV viral suppression, have galvanised efforts worldwide to reduce HIV transmission with the goal of ending the HIV epidemic by 2030. Sex workers, who are particularly vulnerable to HIV, and their sexual partners account for more than half (54%) of new HIV infections globally.1 Available data suggest that the relative risk of HIV...

Chronic renal disease among lead-exposed workers
BackgroundVery high exposure to inorganic lead causes serious kidney damage. We have studied workers with occupational exposure and data on blood lead. MethodsWe extended follow-up for 7 more years, for a previously studied cohort of 58 307 male workers who were part of a surveillance programme in 11 different states. Mortality was assessed using the National Death Index, and end-stage renal disease (ESRD) incidence was assessed using the US Renal Data System. We conducted internal analyses via...

Challenges of pesticide exposure assessment in occupational studies of chronic diseases
In this issue of Occupational and Environmental Medicine, Ohlander and colleagues review methods used to assess pesticide exposure in occupational epidemiology studies over the last 25 years .1 Their findings are striking for two reasons. The first is the very large number of studies published during this time period. Pesticides have been linked to almost every category of chronic disease, including cancer, respiratory, neurological and autoimmune diseases, as well as reproductive outcomes. The authors...

Pneumoconiosis progression patterns in US coal miner participants of a job transfer programme designed to prevent progression of disease
ObjectivesPneumoconiosis prevalence and severity among US coal miners has been increasing for the past 20 years. An examination of the current approaches to primary and secondary prevention efforts is warranted. One method of secondary prevention is the Mine Safety and Health Administration-administered part 90 option programme where US coal miners with radiographic evidence of pneumoconiosis can exercise their right to be placed in a less dusty area of the mine. This study focuses on characterising...

Systematic review of methods used to assess exposure to pesticides in occupational epidemiology studies, 1993-2017
ObjectiveNumerous exposure assessment methods (EAM) exist for investigating health effects of occupational exposure to pesticides. Direct (eg, biomonitoring) and indirect methods (eg, self-reported exposures) are however associated with degrees of exposure misclassification. We systematically reviewed EAM in studies of occupational pesticide exposure. MethodsWe searched for articles reporting observational epidemiological studies in MEDLINE and Embase published 1993 to 2017. The relative frequency...

Severe asthma due to occupational exposure to Guar Gum: is Lipid Transfer Protein involved?
Dear Editor, difficult-to-control asthma may be associated with continuous exposure to allergens, particularly professional ones, presence of comorbidities and lack of adherence to therapy. A component-resolved approach can provide an invaluable support to the diagnosis of allergic asthma.1 On the other hand, molecular diagnosis also underlines new questions, as described here. An otherwise healthy nurse, aged 28 years, experienced onset of asthma symptoms soon after being located to a healthcare...

Firefighter occupation is associated with increased risk for laryngeal and hypopharyngeal squamous cell carcinoma among men from the Greater Boston area
ObjectiveFirefighters are exposed to a wide variety of carcinogens during the line of duty, including several associated with head and neck cancer. Existing studies assessing head and neck cancer risk with firefighting have predominately included occupational cohorts or registry data, which are limited by inability to adjust for smoking and alcohol consumption—major risk factors for head and neck cancer. Our objective was to assess the risk of head and neck cancer among men with an occupational history...
