Prevention of upper aerodigestive tract cancer through active search strategies and use of equipped propaedeutics

Abstract
Introduction

Upper aerodigestive tract cancer is among the most frequent malignancies and has epidemiological importance worldwide. Most cases are already advanced at the diagnosis, with a strong negative impact on survival and high cost to the government. Campaigns directed against these cancers have often failed in Brazil.

Objective

To evaluate the effectiveness of screening for upper aerodigestive tract cancers, using active search strategies and the use of equipped propaedeutics.

Methods

A cross-sectional, prospective, descriptive, analytical and exploratory study, since its objectives are based on the visualization of cancerous lesions in a sample consisting of individuals with risk factors, aiming to expand the necessary knowledge for cancer detection, aiming at secondary prevention of the oral cavity, oropharynx, larynx and hypopharynx cancer.

Results

A total of 16.7% of precancerous lesions and 0.5% of cancer lesions located in the upper aerodigestive tract were clinically visualized.

Conclusion

The method was effective in the identification of precancerous lesions for the purpose of secondary prevention, but equally important against upper aerodigestive tract cancer, since in the present study the chance of finding the latter was increased by 22.7, showing it is an alternative for future campaigns against the disease.

Introdução

O câncer do trato aerodigestivo superior configura-se entre os mais frequentes e apresenta-se com importância epidemiológica no mundo. A maior parte apresenta-se avançada ao diagnóstico, com forte impacto negativo na sobrevida e elevado custo ao erário. As campanhas realizadas contra esses cânceres têm frequentemente falhado no Brasil.

Objetivo

Avaliar a efetividade do rastreamento para lesões cancerizáveis do trato aerodigestivo superior, usando-se estratégias de busca ativa e uso da propedêutica armada.

Método

Estudo transversal, prospectivo, descritivo, analítico e exploratório, uma vez que os seus objetivos se alicerçam na visualização das lesões cancerizáveis numa amostra composta por indivíduos com fatores de risco, de forma a aprofundar o conhecimento necessário para a sua detecção, visando à prevenção secundária do câncer da cavidade oral, orofaringe, laringe e hipofaringe.

Resultado

Foram visualizadas clinicamente 16,7% lesões cancerizáveis e 0,5% cânceres de localização no trato aerodigestivo superior.

Conclusão

O método mostrou-se efetivo na identificação de lesões cancerizáveis com intuito de prevenção secundária, mas igualmente importante contra o câncer do trato aerodigestivo superior, uma vez que neste presente estudo multiplicou-se a chance do encontro deste por 22,7; apresentando-se como alternativa para futuras campanhas contra a doença.

Molecular study of hearing loss in Minas Gerais, Brazil

Abstract
Introduction

Deafness is the most frequent sensory deficit in humans. Incidence is estimated at 4:1000 births in Brazil. Specific programs for clinical care of patients with hearing loss are still scarce in Brazil and the issue is an important public health problem.

Objective

To determine the frequency of 35delG and D13S1830 mutations in GJB2 and GJB6 genes respectively in patients with non-syndromic sensorineural hearing loss from Minas Gerais, Brazil.

Methods

This research involved 53 individuals, who were assessed by a questionnaire for predicting the possibility of non-syndromic deafness and for data collecting. Samples were tested for the presence of the 35delG mutation in GJB2 gene and D13S1830 in GJB6 gene by polymerase chain reaction and restriction enzyme digestion.

Results

Epidemiological research has shown that the majority of the subjects are unaware of the etiology and the pathogenesis of hearing loss. In 9 patients (16.98%), 35delG mutation was found in heterozygosis and the allele frequency was estimate to be around 8.5%. Although 9.61% of the patients reported having some degree of consanguinity between the parents and 12.08% reported other cases of deafness in their families, this mutation was not found in homozygosis. The D13S1830 mutation was not found in this study.

Conclusion

This research describes for the first time the frequency of the 35delG and D13S1830 mutation in hearing-impaired individuals from Minas Gerais, Brazil, and the collected data reinforce the need for further studies in this population due to heterogeneity of hearing loss.

A surdez é o déficit sensorial mais frequente em humanos. Estima-se que a incidência seja de 4: 1000 nascimentos no Brasil. Programas específicos para atendimento clínico de pacientes com perda auditiva são escassos no Brasil e a questão é um importante problema de saúde pública.

Objetivo

Determinar a frequência das mutações 35delG no gene GJB2 e D13S1830 no GJB6 em pacientes deficientes auditivos de origem neurossensorial e não sindrômica de Minas Gerais, Brasil.

Método

A pesquisa envolveu 53 indivíduos selecionados por meio de questionário o qual avaliou a possibilidade de surdez não sindrômica entre outros dados. As amostras foram testadas quanto à presença da mutação 35delG no gene GJB2 e D13S1830 no gene GJB6 por reação em cadeia da polimerase e digestão com enzima de restrição.

Resultados

A pesquisa epidemiológica mostrou que a maioria dos indivíduos desconhece a etiologia da perda auditiva. Em 9 pacientes (16,98%), a mutação 35delG foi encontrada em heterozigose e a frequência alélica foi estimada em 8,5%. Embora 9,61% das pessoas tenham relatado algum grau de consanguinidade entre os pais e 12,08% relatassem outros casos de surdez em suas famílias, essa mutação não foi encontrada em homozigose. A mutação D13S1830 não foi encontrada neste estudo.

Conclusão

Este trabalho descreve pela primeira vez a frequência da mutação 35delG e D13S1830 em deficientes auditivos de Minas Gerais, Brasil, e os dados coletados reforçam a necessidade de mais estudos nesta população devido à heterogeneidade da perda auditiva.

Normative values for singing voice handicap index – systematic review and meta-analysis

Abstract
Introduction

There are no official diagnostic protocols for singing voice assessment. In this publication, on the basis of a literature review, standards for the singing voice handicap index exclusively dedicated to voice disorders in singing have been given.

Objective

The study aims to determine the normative values for the singing voice handicap index.

Methods

The study is a systematic review and a meta-analysis. A systematic literature search was performed using PubMed to access relevant databases and to locate outcome studies. The "inclusion" criteria were as follows: English language, original papers and human studies retrospective and prospective papers, cross-sectional and case-control studies.

Results

Eight articles were included for the final analysis. The normative value for the singing voice handicap index was 20.35 with a confidential range of 10.6–30.1 for a group of 729 healthy subjects whose voices were judged as normal, with an age range of 16–64 years.

Conclusion

The mean normative value of the singing voice handicap index was 20.35 with the confidence levels between 10.6 and 30.1.

Não há protocolos diagnósticos oficiais para avaliação de voz no canto. Nesse estudo baseado em uma revisão de literatura, são fornecidos padrões para o índice de desvantagem vocal no canto exclusivamente dedicado a distúrbios vocais no canto.

Objetivo

O estudo visa determinar os valores normativos para o índice de desvantagem vocal no canto.

Método

O estudo é uma revisão sistemática com meta-análise. Uma busca sistemática da literatura foi realizada utilizando-se o PubMed para acessar bancos de dados relevantes e para localizar estudos de desfechos. Os critérios de inclusão foram os seguintes: estudos escritos em inglês, artigos originais e estudos em seres humanos, retrospectivos e prospectivos, estudos transversais e de caso-controle.

Resultados

Oito artigos foram incluídos na análise final. O valor normativo para o índice de desvantagem vocal no canto foi de 20,35 com intervalo de confiança de 10,6 a 30,1 em um grupo de 729 indivíduos saudáveis cujas vozes foram consideradas normais, com idade de 16 a 64 anos.

Conclusão

O valor normativo médio do índice de desvantagem vocal no canto foi de 20,35 com intervalo de confiança entre 10,6 e 30,1.

Endoscopic cartilage tympanoplasty: full thickness and partial thickness tragal graft

Abstract
Introduction

Cartilage is the grafting material of choice in certain disorders of the middle ear. The indications for its routine use remain controversial due to the possible detrimental effect on post-operative hearing.

Objective

The present study was done to report personal experience with "tragal cartilage shield" tympanoplasty. To compare the results, in terms of graft uptake and hearing improvement, of endoscopic cartilage shield technique of tympanoplasty using either partial thickness or full thickness tragal cartilage for type 1 tympanoplasty and to highlight the tips for single-handed endoscopic ear surgery.

Methods

Fifty patients with safe chronic suppurative otitis media, presenting in our out-patient department from February 2014 to September 2015 were selected. They were randomly allocated into two groups, 25 patients were included in Group A where a full thickness tragal cartilage was used and 25 patients included in Group B where a partial thickness tragal cartilage was used. Audiometry was done 2 months after the surgery for all cases and the patients were followed up for one year.

Results

Out of the total of 50 patients 39 (78%) had a successful graft take up, amongst these 22 belonged to Group A and 17 belonged to the Group B. The hearing improvement in both the groups was very similar.

Conclusion

This study reveals that endoscopic tragal cartilage shield tympanoplasty is a reliable technique; in fact it has a high degree of graft take and good hearing results, irrespective of the thickness. Furthermore, the tragal cartilage is easily accessible, adaptable, resistant to resorption and single-handed endoscopic ear surgery is minimally invasive, sutureless and provides a panoramic view of the middle ear.

A cartilagem é o material de enxerto de escolha no tratamento cirúrgico de certas condições clínicas da orelha média. Devido ao possível efeito prejudicial na audição pós-operatória, as indicações para seu uso rotineiro ainda são controversas.

Objetivo

O presente estudo teve como objetivo relatar a experiência dos autores com a timpanoplastia tipo 1 endoscópica usando cartilagem tragal, e comparar os resultados entre a cartilagem tragal com espessura parcial e espessura total, em termos de integração do enxerto e melhora da audição. O estudo também buscou apresentar sugestões para cirurgia endoscópica de orelha média com uma única mão (single-handed endoscopic ear surgery).

Método

Foram selecionados 50 pacientes com otite média crônica supurativa, atendidos neste ambulatório entre fevereiro de 2014 e setembro de 2015, alocados aleatoriamente em dois grupos: 25 pacientes foram incluídos no grupo A, no qual uma cartilagem tragal de espessura total foi utilizada e outros 25 pacientes foram incluídos no grupo B, no qual foi utilizada uma cartilagem tragal de espessura parcial. Em todos os casos, uma audiometria foi realizada dois meses após a cirurgia; os pacientes foram acompanhados por um ano.

Resultados

Dos 50 pacientes, o enxerto foi bem sucedido em 39 (78%), dentre os quais 22 pertenciam ao grupo A e 17 pertenciam ao grupo B. A melhora da audição em ambos os grupos foi muito semelhante.

Conclusão

O estudo indicou que a timpanoplastia endoscópica com cartilagem tragal é uma técnica confiável, com alto grau de integração do enxerto e bons resultados de audição, independentemente da espessura utilizada. Além disso, a cartilagem tragal é facilmente acessível, adaptável e resistente à reabsorção; a cirurgia endoscópica é minimamente invasiva, sem sutura e proporciona uma visão panorâmica da orelha média.

Intralesional Methotrexate for the Treatment of Keratoacanthoma: The Neapolitan Experience

Abstract

Introduction

Keratoacanthomas are cutaneous neoplasms known for their rapid growth and spontaneous regression over a long time period. Their treatment can be difficult because of the potentially large field size and number of lesions. Intralesional methotrexate constitutes an effective, nonsurgical treatment of keratoacanthomas, as proven by our experience.

Methods

We treated 11 elderly patients affected by keratoacanthoma with intralesional methotrexate. The injections were performed weekly, followed by 10 mg of folic acid to be taken 24 h later.

Results

All our patients underwent complete resolution of the lesions after 4–8 injections, without side effects.

Conclusion

Intralesional methotrexate seems to be an effective and safe nonoperative treatment modality for keratoacanthoma, especially when it arises in anatomic areas difficult to treat with surgery, in elderly debilitated patients, and in those refusing surgery.

Successful Treatment of Generalized Eruptive Keratoacanthoma of Grzybowski with Acitretin

Abstract

Introduction

Keratoacanthomas (KA) are common cutaneous skin tumors originating from the hair follicles. Unlike squamous cell carcinoma, KA can regress spontaneously and have a benign evolution. Solitary KA is the most common form but familial multiple KA (Ferguson–Smith type), genetically predisposed KA (such as in xeroderma pigmentosum, or Muir–Torre syndrome), or sporadic multiple eruptive KA (Grzybowski type) have been described. Generalized eruptive KA of Grzybowski (GEKA) is a rare condition (around 40 reported cases). The pathophysiology is still unclear. Human papillomavirus (HPV) has been detected in sporadic KA but the presence of HPV39 has never been reported, to our knowledge, in GEKA.

Case Report

GEKA in an 80-year-old woman was successfully treated with acitretin (0.5 mg/kg/day) combined with surgical removal of the largest lesions. Treatment was well tolerated and led to decreased pruritus and tumor regression within 6 months. The presence of HPV39 was detected in a lesion by polymerase chain reaction and Sanger sequencing. No genetic alteration was found, in particular in the genes usually altered in squamous cell carcinoma (including NOTCH1, NOTCH2, CDKN2A, TP53).

Conclusion

We report a case of GEKA associated with the presence of HPV39 and the successful use of acitretin combined with surgical removal of the larger lesions.
