Abstract
It is widely accepted that middle ear pathology can cause dizziness and imbalance, however this has been a challenging area to investigate, as the bi-thermal caloric test is difficult to interpret in this cohort. Recently vestibular tests that bypass the middle ear have been developed. These are the Video Head Impulse Test (vHIT) and the Bone Conduction Vestibular Evoked Myogenic Potentials (BC-VEMPs). We have successfully carried out pre-operative vHIT and BC-VEMPs on 20 patients awaiting surgery for middle ear pathologies and have shown that complete testing can be done in less than 30 minutes. We have found that using the minishaker, large BC-VEMP responses can be generated making this technique particularly useful in patients with chronic suppurative otitis media (CSOM) in whom normal air-conduction VEMPs would be abolished. The test results indicate that by using a combination of vHIT and VEMP, abnormal findings were found in 16 of the 40 ears tested (40%) This article is protected by copyright. All rights reserved.
