Arteriovenous fistulas (AVF) remains the preferred method of vascular access for hemodialysis due to favorable patency rates, low infection rates, and fewer complications.1 In order for an AVF to be used, it requires adequate maturation and should be easily cannulated. In addition to affecting maturation, excess adipose tissue overlying the vessels in obese patients often makes the AVF too deep to cannulate for hemodialysis and can necessitate more extensive procedures in order to access the vessels.
This editorial explores value in relation to plastic surgery and strategies that have been suggested to deliver value-based healthcare. We consider how value is measured, accounting for patient outcomes and experiences, costs and equity, and describe strategies that might improve value, such as outcome-based reimbursement, reporting transparency and high volume specialist centres.
Dear Editor,
Fellowships have become the most structured way to prepare for a plastic surgery consultant career in most chosen sub-specialties. High profile microsurgical fellowships in Europe are few and far between, though new fellowships are sometimes developed and merit highlighting. We would like to take this opportunity to highlight our experience, as we were reconstructive fellows with different subspecialty interests in Glasgow, in the hope that other senior trainees with an interest in reconstructive...
Diabetes mellitus increases the susceptibility of free tissue transplantations to ischemia-reperfusion injury. The aim of this study was to enhance NO bioavailability via exogenous nitric oxide synthase and the substrate L-arginine to attenuate ischemia reperfusion-induced alterations in a type 2 diabetes rodent model.
The knee joint and its surrounding are prone to complications after trauma or elective surgery because of its paucity of pliable soft tissue with higher risk of defects or infections. Despite the well-known different techniques1 as first line treatment in reconstructive surgery, this area nevertheless remains a challenge. The aim of this study was to develop a solution to close small to medium defects of this area with minimal use of extra needed split skin grafts especially those after failed knee...
Hidradenitis suppurativa (HS) is a chronic, debilitating disease where even surgery, which is considered the ‘gold standard’ treatment, often has poor outcomes. With the current COVID-19 pandemic, the authors believe it important to make patients aware of the likely surgical outcomes of non-essential surgery. The authors’ aim is to present the complication rates, healing times and risk of recurrence of HS surgery from a London hospital in order to better consent HS patients accurately and offer some...
Clinical governance is the structured approach to maintaining and improving the quality of patient care and is a vital part of global surgery. BFIRST and BSSH closely collaborate with local doctors on a number of overseas projects, seeking to strengthen and develop local knowledge and skills, aiming for an independent local practice in reconstructive and upper limb surgery.Thoughts on essential requirements, improvements and pitfalls in the ethical approach to global collaboratives are presented.
