Anaphylaxis to drugs: overcoming mast cell unresponsiveness by fake antigens
Abstract Our understanding of IgE‐mediated drug allergy relies on the hapten concept, which is well established in inducing adaptive reactions of the immune system to small molecules like drugs. The role of hapten‐carrier adductsin re‐challenge reactions leading to mast cell degranulation and anaphylaxis is unclear. Based on clinical observations, the speed of adduct formation, skin and in‐vitro tests to inert drug molecules, a different explanation of IgE‐mediated reactions to drugs is proposed:...

Component‐resolved microarray analysis of IgE sensitization profiles to Culicoides recombinant allergens in horses with insect bite hypersensitivity
Abstract Background Allergy to bites of blood sucking insects, including biting midges can affect both human and veterinary patients. Horses are often suffering from an IgE‐mediated allergic dermatitis caused by bites of midges (Culicoides spp) . With the aim to improve allergen immunotherapy (AIT) numerous Culicoides allergens have been produced as recombinant (r‐) proteins. This study aims to test a comprehensive panel of differently expressed Culicoides r‐allergens on a cohort of IBH‐affected...

A brief introduction to Climate Change and Health
Abstract Our climate is changing at an unprecedented rate. This not only has impacts on the planet and the ecosystem, with extinction rates rising rapidly, but also on human health (Figure 1). The effects of climate change are and will continue to disrupt the basic requirements for health such as clean water, clean air, adequate food and will furthermore exacerbate underlying social, economic, and ecological

Skin testing and oral amoxicillin challenge in the outpatient allergy and clinical immunology clinic in penicillin-allergic pregnant women
Penicillin allergy is commonly reported. In pregnant women, reported penicillin allergy is associated with negative health outcomes and suboptimal group B streptococcal prophylaxis. For penicillin allergic individuals, skin testing followed by observed oral challenge is recommended. Prior data demonstrate a low risk of adverse reaction with skin testing in pregnant women, but subsequent oral challenge was not routinely pursued.

Patient‐reported sleep outcomes lack association with mucosal eosinophilia or neutrophilia in patients with chronic rhinosinusitis undergoing functional endoscopic sinus surgery
Background Chronic rhinosinusitis (CRS) is associated with sleep dysfunction, but the underlying pathophysiology is poorly understood. The purpose of this study was to determine if mucosal eosinophilia or neutrophilia were associated with sleep dysfunction severity or altered the improvement in sleep dysfunction following functional endoscopic sinus surgery (FESS). Methods A total of 104 patients with medically refractory CRS with nasal polyposis (CRSwNP) and CRS without nasal polyposis (CRSsNP),...

Encephalitis in a Patient With Melanoma Treated With Immune Checkpoint Inhibitors: Case Presentation and Review of the Literature
After the approval of immune checkpoint inhibitors for the treatment of many solid tumors, a new class of adverse events was recognized through the augmented activation of T cells, known as immune-related toxicities (immune-related adverse events). Encephalitis as an immune-mediated phenomenon is extremely rare, but potentially fatal, and questions remain with regard to its optimal management. Herein, we describe a patient with metastatic melanoma who developed autoimmune encephalitis following treatment...

A Phase I, Dose Escalation, Single Dose Trial of Oral Attenuated Salmonella typhimurium Containing Human IL-2 in Patients With Metastatic Gastrointestinal Cancers
Salmonella has been shown to preferentially colonize solid tumors. It is known that toxicity limits the systemic administration of immunomodulatory cytokines that have a significant anticancer effect. Therefore, we tested a unique cancer treatment strategy comprised of oral delivery of Saltikva, an attenuated strain of Salmonella typhimurium that contain the human gene for interleukin-2. In preclinical experimentation, a significant antitumor effect without toxicity was observed. A dose escalation,...

Laryngeal Tuberculosis in a Patient on Avelumab for Metastatic Nasopharyngeal Carcinoma
Increasing numbers of head and neck cancer patients are placed on immune checkpoint inhibitors for indications such as recurrent and metastatic disease. There is a theoretical increased risk of contracting and reactivation of tuberculosis (TB) with programmed cell death-1 blockade due to potentiation of type 1 T helper response and increased production of interferon-gamma. This is a potentially life-threatening complication of therapy and requires expedient diagnosis and treatment. We present a case...
